2. Payroll Summaries
Payroll Summaries for each affected quarters. Make sure they are divided into quarters so the calculations for each employee is correct and accurate.
There are quarterly state wage reports that should have been included in your returns. These reports provide all the information that we need in regards to the quarterly payroll summaries. We will provide the specific name of the report that corresponds to your state upon request.
Listed below are the dates for each quarter in 2020 and 2021 that qualify for the ERC program.
Mar 13, 2020, to Mar 31, 2020 [Q1]
April 1, 2020, to June 30, 2020 [Q2]
July 1, 2020, to Sept 30, 2020 [Q3]
Oct 1, 2020, to Dec 31, 2020 [Q4]
Jan 1, 2021, to Mar 31, 2021 [Q1]
Apr 1, 2021, to June 30, 2021 [Q2]
Jul 1, 2021, to Sept 30, 2021 [Q3]
Oct 1, 2021, to Dec 31, 2021 [Q4] *(ONLY IF START-UP BUSINESS)*
State Unemployment Insurance Fillings (SUI - which has gross employee wages for each affected quarter) could be used in place of the Payroll Summaries. We do not need to see any sensitive information like SSN or Address. That can all be left off the reports or blacked out.

~ Type a 2 to 3 sentence statement detailing how COVID-19 affected your business
